Certain sections are controlled by signal lights. Oversight comes from a board in Texas monitorinh each train. An axle sometimes has brakes fail, called a hotbox, this creates melting steel. A mass failure at 40 would take at least a miles to stop. Longer miles if faster. If a caboose is attached the conductor should have sen the hotbox sparks. If just abflashing freddie on tail end. NOone would see unless engineer looked back. This was a planned event in my opinion.
